In this case study we see the piriformis muscles are markedly asymmetric, being larger right than left. At the level of the sciatic notch an anatomical variant is appreciated in that there is a split piriformis muscle and split sciatic nerve on the right.
The sciatic nerve components pass partially through the piriformis muscle. The sciatic nerve demonstrates a significant increase in image intensity as it traverses the split within a more significant alteration affecting several fascicles quite prominently as the nerve rejoins its components.
There is also severe and extensive trochanteric bursitis with extensive inflammation into the surrounding tissues.
